This notice describes how medical information about you may be used and disclosed and how you can get access to this information.

Authorization for Disclosure of Protected Health Information

Patients may request a copy of their own medical and billing records by coming into the Administration Office with a valid photo ID and a completed Authorization for Disclosure of Protected Health Information. For more information contact Office Manager Teresa Weiss at tweiss@uprad.org

Open Records Request Policy and Form
